Community Outreach Services Mental Health Nurse/Allied Health Professional 

Part-Time: 3 days per week 

About The Role:

The Community Outreach Service clinicians are responsible for the delivery of community-based services to people with mental health conditions as part of a broader hospital system. Delmont is seeking a self-motivated individual, with a vision for excellence and a strong desire and commitment to provide quality care and service to people with complex mental health issues.

Required Skills and Experience:

  • Knowledge and experience working with a variety of therapeutic modalities; ACT, CBT, Motivational Interviewing or Solution Focused Therapy
  • Deliver high-quality clinical care: Participate in COS referral, triage and admission processes, provide discipline-specific assessment and treatment, and manage an allocated clinical caseload including outreach visits.
  • Collaborate in person-centred care planning: Work within a multidisciplinary team to plan, implement and review individual treatment and discharge plans, offering specialist consultation to consumers, carers, medical staff and external partners.
  • Support consumers and families: Provide psychoeducation and practical support to consumers and their families/significant others, with a strong focus on recovery, relapse prevention and community connection.
  • Maintain professional and organisational standards: Ensure accurate and timely clinical and administrative documentation, contribute to quality improvement initiatives, meet legislative and policy requirements, and engage in ongoing professional development and supervision.
  • A minimum of 5 years’ experience working in mental health
  • Victorian Drivers Licence
  • An ability to work well autonomously and as part of a team

Renumeration:

  • Salary will be commensurate with qualifications and experience and as per the relevant Delmont EA.

Requirements:

  • All Delmont Hospital Employees are required to be vaccinated against COVID-19 and Influenza with a TGA approved vaccine and must provide evidence of their vaccination status. This is in accordance with the legislated requirement for influenza vaccination.
