Community Outreach Service

Delmont Community Outreach Service is an important part of the overall range of services provided by the hospital. Our experienced mental health staff offer a home or community based service. Our program is designed for people who are:

  • Experiencing difficulty making the transition from hospital based treatment to home.
  • Facing challenges in coping with their illness and treatment.
  • Experiencing disharmony with their family and social relationships due to mental illness
  • Currently being treated by a Psychiatrist with Delmont Private Hospital.

Our aims are to:

  • Provide a comprehensive assessment of your mental health needs.
  • Develop an individual treatment plan which supports your needs
  • Provide education and advice to patients and carers on identified issues such as stress/ anxiety management and depression.
  • Promote the exploration of community networks and support services.
  • Support your personal growth towards a more independent balanced lifestyle.
  • Work in a collaborative manner with your treating Doctor and liaise with other appropriate health services.

How do I access the Community Outreach Services

A referral from your Delmont Treating Doctor is required

A member of the Community Outreach Service will then contact you to discuss your referral.

Cost

Private Health insurance may cover the cost of the Community Outreach Service visits. A fee-paying service is available for those without private health cover.

The Community Outreach Services operates between the hours of 8:30am - 4:30 pm, Monday to Friday
